Patient self reporting of adverse drug reactions
Patients who suspect they have suffered an unwanted side effect (an adverse drug reaction) to their medicines can now report this, as can their carers or
parents, to the MHRA directly, or by telephone on 0808 100 3352. Alternatively paper versions of the yellow card, in leaflet form, are available from many NHS outlets including GP surgeries and community pharmacies.
